ABSTRACT

Introducción: El sarcoma de Kaposi es una neoplasia maligna multifocal de células endoteliales, que se presenta con lesiones características en la piel, boca y ganglios linfáticos. Objetivo: Describir un caso de sarcoma de Kaposi que, por sus características, entorpeció el diagnóstico de infección por VIH. Presentación del caso: Paciente masculino, de 30 años, diagnosticado recientemente con síndrome de inmunodeficiencia adquirida, sin tratamiento. Fue referido a la consulta de cirugía maxilofacial para biopsia de adenopatía cervical derecha. Durante el examen físico se observó edema generalizado en ambos miembros inferiores, con lesiones hiperpigmentadas de aspecto macular y papular de varias tonalidades. En la mucosa del paladar duro se apreció una lesión pigmentada violácea, que abarcaba la línea media palatina y se extendía hacia la hemiarcada izquierda. Se realizó la exégesis de la adenopatía con fines diagnósticos, con lo cual se confirmó sarcoma de Kaposi. Conclusiones: La falta de familiaridad con las características clínicas, así como la falta de una biopsia de las lesiones cutánea y un examen completo de la cavidad oral, ocasionaron que esta neoplasia pasara desapercibida en los exámenes médicos de rutina en el caso presentado, lo que condujo a un bajo índice de sospecha de la infección por VIH/sida y retrasó el tratamiento oportuno. Cuando la inmunosupresión está avanzada, el sarcoma de Kaposi relacionado al síndrome de inmunodeficiencia adquirida tiende a ser más agresivo(AU)


Introduction: Kaposi sarcoma is a multifocal malignant neoplasm of endothelial cells which presents with characteristic lesions on the skin, mouth and lymph glands. Objective: Describe a case of Kaposi sarcoma with characteristics that hindered HIV infection diagnosis. Case presentation: A case is presented of a male 30-year-old patient recently diagnosed with acquired immunodeficiency syndrome, and not following any treatment. The patient was referred to the maxillofacial surgery service for right cervical adenopathy biopsy. Physical examination revealed generalized edema in both lower limbs, with hyperpigmented lesions of macular and papular appearance and various shades of color. A purplish pigmented lesion was detected in the hard palate mucosa. The lesion covered the palatal midline and extended toward the left hemiarch. Exeresis of the adenopathy was performed for diagnostic purposes, confirming the diagnosis of Kaposi sarcoma. Conclusions: Lack of familiarity with the clinical characteristics of Kaposi sarcoma, and not performing a biopsy of the skin lesions and a complete examination of the oral cavity, made the neoplasm go unnoticed to routine medical tests, leading to low suspicion of HIV/AIDS infection and delaying timely treatment. In advanced immunosuppression, Kaposi sarcoma related to acquired immunodeficiency syndrome tends to be more aggressive(AU)

Introducción: El tumor tritón maligno es una neoplasia rara en la que se encuentran células rabdomioblásticas en un tumor maligno de la vaina de nervios periféricos, que se caracteriza por su agresividad y mal pronóstico. La localización en la cabeza y el cuello es poco frecuente. La inmunohistoquímica juega un papel importante en el diagnóstico. Objetivo: Describir un tumor tritón maligno de tamaño inusual. Presentación del caso: Paciente femenino, de 16 años, es referida al servicio de cirugía maxilofacial del Instituto Nacional de Pediatría, Ciudad de México, con un diagnóstico de tumor neuroectodérmico en región facial y cervical de un año de evolución. Clínicamente el tumor era exofítico, multilobulado, con zonas extensas de necrosis, superficie de varias tonalidades y un tamaño aproximado de 18 x 10 x 12 cm. Se realizó una biopsia e inmunohistoquímica que confirmó el diagnóstico de tumor tritón maligno. La paciente fue intervenida quirúrgicamente, procedimiento con el cual se eliminó totalmente la lesión, con márgenes de seguridad. La paciente presentó una evolución tórpida, con desenlace fatal al cabo de seis meses del tratamiento. Conclusiones: El tumor tritón es una neoplasia agresiva y su detección oportuna orienta al cirujano a ofrecer al paciente un tratamiento adecuado(AU)


Introduction: Malignant triton tumor is a rare neoplasm in which rhabdomyoblasts are present in a malignant tumor of the peripheral nerve sheath. This condition is characterized by its aggressiveness and bad prognosis. Location in the head and neck is infrequent. Immunohistochemical testing plays an important role in its diagnosis. Objective: Describe an unusually large malignant triton tumor. Case presentation: A case is presented of a female 16-year-old patient referred to the maxillofacial surgery service of the National Institute of Pediatrics in Mexico City with a diagnosis of neuroectodermal tumor of one year's evolution in the facial and cervical region. In clinical terms, the tumor was exophytic, multilobed, with extensive areas of necrosis, a surface in several shades of color and an approximate size of 18 x 10 x 12 cm. Biopsy and immunohistochemical testing confirmed the diagnosis of malignant triton tumor. The patient underwent surgery in which the lesion was totally excised with a safety margin. Evolution was clumsy, with a fatal outcome at six months of treatment. Conclusions: Triton tumor is an aggressive neoplasm whose early detection makes it possible for surgeons to provide an appropriate treatment(AU)

SUMMARY: The purpose of this study was to determine the shape and diameter of the physiological foramen in maxillary premolars of a Chilean population. The sample consisted of 125 extracted maxillary premolars, in which the apical three to five millimeters were cut and observed under magnification of 40x, photographed (Motic Cam), and analyzed using Motic Images Plus 2.0 ML in order to obtain measurements of the minor and major diameter of each physiological foramen. The average minor and major diameter in maxillary premolars was 0.270 and 0.413 mm, respectively. In terms of shape, 72.19 % had oval shaped physiological foramen; 18.93 % were irregularly shaped and 8.88 % were round. Oval and irregular canals are significantly more difficult to shape and require special attention in order to obtain a thoroughly disinfected root canal system.


RESUMEN: El objetivo del estudio fue determinar la forma y el diámetro del foramen fisiológico de premolares maxilares en una población Chilena. La muestra consistió en 125 premolares maxilares extraídos, a los que se cortó de 3 a 5 milímetros apicales, fueron observados bajo magnificación de 40x, fotografiado (Motic Cam) y analizados utilizando Motic Images Plus 2.0 ML para obtener las mediciones de el diámetro menor y mayor de cada foramen fisiológico. El promedio del diámetro menor y mayor en premolares maxilares fue de 0,270 y 0,413 mm respectivamente. En cuanto a la forma 72,19 % tenía el foramen fisiológico de forma oval; 18,93 % tenía forma irregular y 8,88 % fue redondo. Canales ovales e irregulares son significativamente más difíciles de conformar y requieren especial atención para obtener una adecuada desinfección del sistema de canales radiculares.

ABSTRACT

Introducción: el carcinoma de células escamosas primario intraóseo es una neoplasia maligna poco frecuente. Objetivo: presentar un caso clínico de un carcinoma primario intraóseo derivado de un quiste odontogénico en las regiones del cuerpo a la rama mandibular izquierdas. Caso clínico: acude a consulta paciente masculino de 68 años quien refiere una molestia en la mandíbula. Al examen bucal se detecta expansión cortical a nivel del 37. Se realiza estudio de ortopantomografía donde se observó un área radiolúcida con borde definidos, y bajo la impresión diagnóstica de quiste residual se efectúa curetaje y estudio histopatológico, que confirma lo sospechado. Al cabo de unos 4 meses el paciente acude nuevamente refiriendo dolor muy intenso en la región del ángulo mandibular con ligera parestesia del labio inferior izquierdo que se asociaba al examen físico facial, con edema geniano bajo y submandibular, y al examen bucal con movilidad anormal ósea mandibular izquierda. Se realiza ortopantomografía donde se observa imagen radiolúcida de bordes irregulares, poco precisos, y fractura patológica mandibular. Se decide inmovilización y toma de muestra para biopsia, que informa un carcinoma epidermoide moderadamente diferenciado, por lo que se realiza tratamiento quirúrgico radical con manejo del cuello, más radioterapia y quimioterapia posoperatorias. Conclusión: la transformación en un carcinoma primario intraóseo a partir de un quiste residual es una entidad infrecuente en la región maxilofacial. El diagnóstico en el caso presentado se realizó por exclusión. La sintomatología de dolor intenso y parestesia fueron elementos clave en la sospecha diagnóstica. El enfoque oncológico mediante cirugía radical con manejo del cuello, radio y quimioterapia permitieron el control loco-regional(AU)


Introduction: primary intraosseous squamous cell carcinoma is an infrequent malignant neoplasm. Objective: present a clinical case of primary intraosseous carcinoma arising from an odontogenic cyst in body regions to the left mandibular branch. Clinical case: a 68-year-old male patient presented with discomfort in his mandible. Oral examination revealed cortical expansion at the level of 37. Orthopantomography showed a radiolucent area with definite borders. Curettage and histological testing confirmed the preliminary diagnosis of residual cyst. Some four months later the patient presented again with very intense pain in the mandibular angle region with slight paresthesia of the left lower lip area associated on physical facial examination with lower and submandibular genian edema, and on oral examination with abnormal left mandibular bone mobility. Orthopantomography revealed a radiolucent image of irregular, imprecise borders and pathological mandibular fracture. It was decided to immobilize and take a sample for biopsy, which reported moderately differentiated epidermoid carcinoma. Therefore, radical surgical treatment was performed with neck management plus postoperative radiotherapy and chemotherapy. Conclusion: evolution of a residual cyst into a primary intraosseous carcinoma is an infrequent condition in the maxillofacial region. In the case herein presented diagnosis was made by exclusion. The symptoms of intense pain and paresthesia were crucial to the preliminary diagnosis. Application of an oncological approach based on radical surgery with neck management and radio- and chemotherapy resulted in local-regional control(AU)

ABSTRACT

The prevalence of overweight and obesity is increasing, creating a public health problem. The loss of approximately 10% of body weight is recommended to reduce the risk of mortality associated with metabolic diseases and to increase the quality of life in adults with overweight or obesity. Non-pharmacological strategies used for weight management are caloric restriction and physical exercise. Nevertheless, the independent effect of physical exercise to decrease body weight is unclear, and could be responsible for only 20% of the weight loss when healthy lifestyles are prescribed. However, exercise has other benefits for health, independent of its weight reducing effect. In fact, physical inactivity is responsible for twice the deaths caused by obesity. The aim of this review is to discuss the importance of physical exercise in the reduction of body weight in subjects with overweight or obesity.

ABSTRACT

El quiste óseo aneurismático es definido como una lesión osteolítica expansiva que consiste en espacios llenos de sangre y canales divididos por tabiques de tejido conectivo, los cuales contienen tejido osteoide y células gigantes multinucleadas. El objetivo es presentar un caso clínico poco común de un quiste óseo aneurismático de la región del cuerpo mandibular. Se trata de una paciente femenina de 39 años de edad que acudió a consulta externa del Servicio de Cirugía Maxilofacial del Hospital Universitario General Calixto García por aumento de volumen en región mandibular derecha y dolor intenso de 1 mes de evolución. Radiográficamente se detectó un área radiolúcida unilocular de bordes bien definidos; se realizó curetaje de la cavidad, y estudio histopatológico de la lesión que informó la presencia de un quiste óseo aneurismático. Se concluye que el quiste óseo aneurismático es más común en los huesos largos y en la región del ángulo mandibular en el esqueleto facial, por lo que la presentación de este en el cuerpo mandibular resulta de interés(AU)


The aneurysmal bone cyst is defined as an expansive osteolytic lesion consisting of blood-filled spaces and divided by partitions of connective tissue, which contain bone tissue and giant cells channels. The objective is to present a rare case of an aneurysmal bone cyst in the region of the mandibular body. This is a case of a 39-year-old female who attended the outpatient Maxillofacial Surgery Service of General Calixto Garcia University Hospital due to an increased volume in right mandibular region and intense pain of a month, both in evolution. Radiographically, a unilocular radiolucent area with well-defined edges was detected; curettage of the cavity and histopathological examination of the lesion were performed, which reported the presence of an aneurysmal bone cyst. It is concluded that the aneurysmal bone cyst is most commonly presented in the long bones and in the region of the mandibular angle in the facial skeleton, so the presentation of this in the mandibular body is of interest(AU)

Background: Short term physical training programs may improve insulin resistance and hyperglycemia. Aim: To assess the effects of eight weeks of combined exercise program on serum lipids and glycemic level in women with hyperglycemia and hypercholesterolemia. Patients and Methods: Ten healthy women, nine women with hyperglycemia, ten with hypercholesterolemia and nine with hyperglycemia/hypercholesterolemia were studied. Participants were subjected to eight weeks into a program of combined physical exercise (high intensity interval + resistance training). Results: Fasting glycemia decreased by 12 and 14% in hyperglycemic and hyperglycemic/hypercholesterolemic participants, respectively. Serum insulin decreased in all groups in a range from 27 to 37%. HOMA IR for insulin resistance decreased similarly. A significant decrease in TC and TG was observed only in those altered baseline subjects. Conclusions: Eight weeks of combined physical exercise had a favorable effect on insulin resistance in this group of women.

Background: High intensity training could be an effective way of improving health on individuals at high metabolic risk. Aim: To investigate the effects of a high intensity training intervention on metabolic-related markers in sedentary women at high metabolic risk. Material and Methods: Forty six sedentary women with a body mass index (BMI) over 25 kg/m² were assigned to four groups, according to their metabolic profile; hyperglycemia (H, n = 12), hyperglycemia/hypercholesterolemia (HH, n = 13), normoglycemia (N, n = 10) and normoglycemia/hypercholesterolemia (NH, n = 11). For 12 weeks and five days per week, subjects performed seven intervals of high intensity training (20 to 30 seconds) during a training session of 20 minutes. Anthropometric (body weight, body mass index (BMI), waist circumference) and metabolic variables (glucose, total cholesterol, LDL, HDL and TG) were measured at baseline, at 6 and 12 weeks of intervention. Results: BMI and waist circumference decreased significantly after 12 weeks of intervention. Similarly, glucose decreased significantly after 12 weeks of intervention in all groups. The reduction was of higher magnitude in those groups with hyperglycemia (H = -16%, HH = -22%, N = -7,5%, NH = -9,6%). However, lipid profile (TG, total cholesterol, LDL and HDL) improved significantly only in the hypercholesterolemic groups. Conclusions: Physical activity programs incorporating high intensity training can improve glucose and lipid profile in women with metabolic disorders. Moreover, this benefit is greatest in those individuals with highest metabolic burden.
